On the record
What the VA Office of Inspector General has published about this facility, newest first. The summaries are ours; the reports are theirs.
- Healthcare Facility Inspection of the VA Orlando Healthcare System in Florida (2024-09-24) — Cyclical inspection (site visit the week of February 12, 2024) found inpatient beds kept above approved levels on two units after the pandemic without the required bed-change process, and an unsecured medication tube system in the locked dementia unit; two recommendations, and the facility was otherwise found clean and well maintained.
- Comprehensive Healthcare Inspection of the Orlando VA Healthcare System in Florida (2021-11-03) — CHIP evaluation of the system’s inpatient and outpatient care found patient-experience scores for access to urgently needed appointments generally lower than VHA national averages, and concerns with identifying sentinel events and completing institutional disclosures; four recommendations in three areas.
- Delays in Processing Community-Based Patient Care at the Orlando VA Medical Center, Florida (2019-02-20) — Requested by Rep. Bill Posey after a patient died while awaiting approval for outside heart surgery; the OIG did not substantiate that the death was caused by the approval delay, but substantiated processing delays across other thoracic surgery community-care consults in a 10-month period in 2017 and made six recommendations.
- Concerns Related to the Management of a Patient’s Medication at Three VA Medical Centers and Inaccurate Response to a Congressional Inquiry at the VA Illiana Health Care System, Danville, Illinois (2019-01-16) — Reviewing one patient’s care across three medical centers, the OIG found that in 2012 an Orlando VA provider did not tell the patient about an abnormal electrocardiogram or arrange follow-up, and that no provider documented discussing the risks of the patient’s 300 mg amitriptyline dose; the eight recommendations included Orlando’s compliance with test-result notification and follow-up.
- Combined Assessment Program Review of the Orlando VA Medical Center, Orlando, Florida (2014-05-06) — CAP review (site visit the week of March 24, 2014) made recommendations in four of the nine areas reviewed: late professional practice evaluations, unsecured medication and supply carts, controlled-substance inspection gaps, and undocumented residential-program inspections and bed checks.
