On the record
What the VA Office of Inspector General has published about this facility, newest first. The summaries are ours; the reports are theirs.
- Review of Inpatient Mental Health Unit Processes at the West Palm Beach VA Healthcare System in Florida (2026-06-04) — Following congressional inquiries after a patient death caused by another patient in March 2024, OIG found staff had not followed the Mental Health Environment of Care Checklist for a wheelchair patient’s room assignment and observation, repeating findings from 2019; six recommendations issued.
- Inadequate Coordination of Care for a Patient at the West Palm Beach VA Healthcare System in Florida (2023-03-30) — OIG did not substantiate that providers failed to coordinate a patient’s cancer care, but found the pulmonologist did not inform the patient of CT scan results or use the required scheduling process, and a facility chiropractor did not act on a community-care MRI recommendation; three recommendations issued.
- Healthcare Inspection – Quality of Care Issues, West Palm Beach VA Medical Center, West Palm Beach, Florida (2014-12-18) — OIG substantiated that the wrong lens was implanted in a cataract surgery after a failed time-out, that incident-reporting policy was not followed, and that Quality Management was chronically understaffed; four recommendations issued.
