On the record
What the government’s own watchdogs and prosecutors have published about this company, newest first. The summaries are ours; the findings are theirs.
- VA Disability Benefits: Additional Oversight and Information Could Improve Quality of Contracted Exams for Veterans (GAO-25-107483) (2025-08) — GAO: in FY2024 contractors conducted over 3 million exams costing over $5 billion; the VA had no written procedures to verify incentive calculations and incorrectly paid almost $2.3 million in incentives in Q1 FY2024; focused reviews of complex claims (TBI, military sexual trauma, Gulf War illness) were behind schedule.
- Better Oversight Needed of Accessibility, Safety, and Cleanliness at Contract Facilities Offering VA Disability Exams (VA OIG 23-01059-72) (2024-05-08) — OIG inspectors found one or more ADA/OSHA deficiencies at 114 of 135 contract exam facilities visited; the VA relied on the vendors to self-certify compliance.
- Contract Medical Exam Program Limitations Put Veterans at Risk for Inaccurate Claims Decisions (VA OIG 21-01237-127) (2022-06-08) — The OIG found the exam vendors then under contract (VES, QTC and LHI/Optum Serve) consistently below the contracts’ 92% exam-accuracy requirement since at least 2017; errors the VA identified were not corrected before claims decisions, and the contractual monetary disincentives were not used.
