Audit of the Healthcare Enrollment Program at VA Medical Facilities
VA Office of Inspector General · Report 24-03691-175 · August 21, 2026 · Office of Audits and Evaluations
What the OIG found
The OIG audited whether facilities processed health care enrollment applications consistently with national standardization policies, reviewing records created from October 2022 through November 2025 amid a post-PACT Act surge and procedures through January 2026. Staff processed 75 percent of 420 sampled applications correctly, but 14 percent were not maintained or date-stamped so timeliness and accuracy could not be verified, and errors in interpreting, overlooking, or entering information produced wrong eligibility or priority group determinations in 10 percent. Some facility practices also kept or delayed applications from ever entering the enrollment system, a gap quality assurance missed because it only looked at completed applications. The OIG made eight recommendations, and the acting under secretary for health concurred with all of them.
Who it concerns
VA medical facilities — Healthcare Enrollment Program processing of veterans’ health care enrollment applications
Why it matters to a family
Enrollment is the front door to VA health care — a mis-filed application or a wrong priority group can delay or block the care a veteran’s family is counting on, including care tied to PACT Act eligibility.
