VA Office of Inspector General · Report 25-00203-180 · September 30, 2026 · Office of Healthcare Inspections

What the OIG found

The OIG carried out a focused inspection of care at the Central Arkansas Veterans Healthcare System in Little Rock, looking at culture, environment of care, patient safety, integrated veteran care, and the veteran-centered safety net. Problems surfaced in only one area, the environment of care. The OIG issued three recommendations, all in that area, calling on the director to assess where temperature and humidity monitoring is needed, to address liquid nitrogen tank storage and keep access to exits clear, and to prevent previously identified findings from recurring.
