Healthcare Facility Inspection of the VA North Texas Health Care System in Dallas
VA Office of Inspector General · Report 26-00055-01 · October 8, 2026 · Office of Healthcare Inspections
What the OIG found
OIG conducted a focused Healthcare Facility Inspection of the Dallas system, the highest-complexity type of facility, examining care delivery and patient safety across five domains: culture, environment of care, patient safety, integrated veteran care, and the veteran-centered safety net. Inspectors looked at everything from employee and veteran experience and the cleanliness and safety of patient care areas to test-result communication processes, staffing, access to primary and community care, and support programs for vulnerable veterans. The inspection did not reveal issues in any of the five domains that warranted recommendations for corrective action. OIG made zero recommendations and closed the report on October 8, 2026.
