Healthcare Facility Inspection of the VA Shreveport Healthcare System in Louisiana

VA Office of Inspector General · Report 26-00057-170 · October 2, 2026 · Office of Healthcare Inspections

What the OIG found

OIG inspected the Shreveport system from March 23 through 26, 2026, with an update in June 2026 and primary care wait time data reviewed through July 2026, examining a mid-high-complexity facility that provided direct care to about 39,700 patients in fiscal year 2025 across five domains: culture, environment of care, patient safety, integrated veteran care, and the veteran-centered safety net. Both recommendations came from patient safety: the facility lacked service-level workflows outlining staff roles in communicating test results, and the Chief of Staff and Associate Director for Patient Care Services did not consistently review performance data on test result communications or take corrective action when abnormal results were not communicated within seven days as VHA Directive 1088(1) requires. No recommendations arose in the other four domains, including environment of care and access to primary and community care. OIG recommended the two leaders develop written workflows for test-result communication and routinely review the performance data and correct noncompliance with the seven-day standard for abnormal results.

Who it concerns

VA Shreveport Healthcare System in Louisiana

Why it matters to a family

For families, the seven-day rule for abnormal test results is the point: a veteran’s family should not learn weeks later that a result requiring action sat uncommunicated, and this facility had no reliable system to catch that failure.
